H1N1 FLU SHOT CLINIC
PINNACLE HIGH SCHOOL - SATURDAY - DECEMBER 5th - 9AM-2PM IN THE GYM
THIS CLINIC OFFERS BOTH THE SEASONAL AND H1N1 FLU SHOT AND IS ONLY FOR PINNACLE STUDENTS AND STAFF!
Pre-register your children for an H1N1 vaccination at our school by following the steps out-lined below.

3. Print out the confirmation / consent form for each of your children
School Clinic Date: Saturday, December 5th.
School Clinic Hours: Vaccinations will be administered from 9am-2pm only.
General Information for Parents:
****Note: High school students do not have a homeroom teacher, please use your English teacher's name.
• Immunizations will be provided by Mollen Immunization Clinics in conjunction with Maricopa County Health Department and our School District
• Most health insurance plans and AHCCCS plans are accepted with no co-payment for the H1N1 immunizations
• No child without insurance will be denied the H1N1 vaccination
• Thorough screening questions are used to ensure that each child is administered the appropriate vaccine
• Registration is currently available in English and Spanish
• If you pre-register your child 48 hours in advance of the school clinic, he/she is not required to bring in a copy of their consent form.
• If you pre-register your child less than 48 hours prior to the school clinic then your child will need to bring in a copy of his/her signed consent form
• You will receive a verification of the vaccine administered via e-mail
• You may select to have the record of the vaccine administered forwarded to your child's Doctor
