Emergency Card
Please be sure we can reach you at all times!!!!
Per district policy, parents/guardians must complete and sign the student emergency card to complete registration and enrollment. Emergency cards for high school students must be completed, signed and returned no later than walk- thru. Please make sure you have answered
ALL questions, as we do not keep emergency cards from the previous year.
IT IS VERY IMPORTANT THAT YOU COMPLETE THE HEALTH CONDITIONS AND MEDICATIONS INFORMATION COMPLETELY TO BETTER SUPPORT YOUR STUDENT. IN AN EMERGENCY THIS INFORMATION IS INVALUABLE.
PLEASE NOTIFY THE HEALTH OFFICE OF ANY HOME, WORK, OR EMERGENCY TELEPHONE NUMBERS OR ADDRESS CHANGES IMMEDIATELY SO WE MAY UPDATE YOUR CHILD'S EMERGENCY CARD.
