Has your child had a recent immunization? All students need to be current with their immunizations. If your child has had an update, please send a copy to the school health office or fax it to us with their name and date of birth to: 602-449-4068
If your child had a Chronic Health Placement or Homebound Placement, be sure to secure paperwork from the school nurse for the 2009-2010 school year. This request must be made annually.
