1. My son/daughter received their immunizations today. Do you need any proof?
Yes. Any time that your child receives any immunizations, the school nurse should always be provided proof. Proof is a stamped or signed shot record from the doctor or facility administering the immunizations or a receipt from the doctor's office.
2. My son/daughter is going to high school in August. Do I need to send them the shot records or do you?
No. If your child is going to a Paradise Valley School District High School, your child's health file will be forwarded to the High School Nurse. If your child is planning to attend a high school outside of our school district it is recommended that you give the next school a copy of your child's shot records since no records will be sent to them unless the new high school does a written request to our school.
3. I got a letter from the school nurse today saying I had 3 weeks to get my child the TDAP but my Doctor says it's not due until they are in High School. Can you tell me why you are telling me it's due?
By following the Arizona State law, the school nurse must require each and every student to aquire their TDAP (or Tetanus booster, as most parents refer to it) 10 years after the students last DTap. Therefore, all the parent has to do is look at their child's shot records and determine when the last DTap was given and add 10 years on to this date and that is where the School Nurse gets the due date from. We do not go by grades in school for this shot. PLEASE NOTE, this law will be changing soon and it WILL BE REQUIRED to receive the TDAP five years after the DTap.
