Registration Requirement Checklist
The following information is needed to register your student.
Please include the area code with any telephone numbers requested.
1. Expulsion/Long Term Suspension Waiver must be completed.
2. Withdrawal Notice is required from student's previous school.
3. Proof of Birth /Identity as required by Arizona Revised Statute 15-828:
o certified copy of birth certificate
o baptismal certificate
4. Immunization History as required by Arizona Revised Statute 15-803:
o complete immunization history from official school record/doctor/clinic.
5. If you are not the legal guardian, you must obtain “Power of Attorney” papers and approval from an administrator at Shadow Mountain High School.
6. If there has been a divorce or legal separation, you are to provide papers to show custody.
7. School transcript, Stanford 9, and AIMS test results.
8. Special Education students need psychological evaluation and IEP (individual education plan) documents and an appointment with Special Ed personnel. Please call the Special Ed. office at 867-5347 to schedule an appointment.
9. Proof of Residency as required by Paradise Valley School District Policy:
o lease agreement or escrow papers;
o utility bill in your name; or
o notarized statement from legal occupant with whom you live, who in turn must show proof of residency through one of the above methods.

· You may fax above information to 602-867-5317 prior to your appointment.
· If you have a problem obtaining the above information, please notify us prior to your appointment.
· If you are unable to keep your appointment, please call the Guidance office at 602-867-5306 or 602-867-5307.
· Please be at least 15 minutes early for your scheduled appointment. Your registration packet needs to be reviewed.
· Lack of necessary documentation and completed packet will require us to reschedule your appointment. Please have all necessary documentation ready at time of registration.
