STUDENT DENTAL SERVICES - MOBILE

CONTRACT SCOPE:

The purpose of this award is to provide a long term contract with a qualified firm to provide on-site student dental services to various elementary and middle schools within the Paradise Valley School District. Initially, the dental services program will be available at all Title I schools, with the option for the program to be activated at any school within the District. The firm has the capability to perform any and all dental procedures within the school itself or through a fully equipped mobile trailer. Students who are AHCCCS eligible will not be charged for any dental treatment. Students who may not qualify for AHCCCS reimbursement will be charged at the discounted AHCCCS reimbursable rate schedule. For example, a student who is not AHCCCS eligible will be charged $59.00 for an initial exam, x-ray, cleaning, and fluoride treatment.
