Athletic passes will be $50, this pass will allow students to attend all sports home games for FREE.
Family Pass - 2 adults/3 kids non high school/2 grandparents- $95.00 school year. With proof of joining the school Booster club the price will drop to $85.00. The Family Pass will be honored by all district high schools but will exclude Invitational, Region and State Tournaments.
Athletic Emergency Cards: These can be picked up in the Athletic Director's Office.
At Horizon we encourage you to participate in as many activities
as you would like. To help you make your decision about athletics,
the following information and the links on this page (to the right) will provide
general information as well as inform you of what sports are
offered, when that session takes place, and who the head coach
is.
To Participate in Athletics:
- All participants need to have an A.I.A. physical on file
with the Athletic Director's office. The physical must be
done after March 1st this year to be used for next year and
also must be signed by a medical doctor.
- All participants must have passed all classes from the
previous quarter, or made up the same number of failed classes
in summer school. (Summer school classes do not have to be
the same as those failed.)
- All participants must pay a participation fee prior to issuance of game equipment.
- In order to participate in District organized athletics, stdents and their parents must view the online Parent consent Sports Injury Video prior to participation. A link to this video can be found at:

- District requirements and dates are subject to change. If you

NOTE: Students who transfer may not be eligible to compete
in athletics upon entering Horizon. Please call the Athletic
Department at 602-449-6023 if you have questions about athletic
eligibility.

you will find what you need on that page.
Who needs a 520 form? Anyone that is transferring to Horizon that is a 10th, 11th, or 12th grader and wants to play sports at Horizon.
Who needs a 530 form? Anyone that filled out a 520 form and played sports at their last school.
