Mollen Clinic
The Mollen Clinic will be on campus THURSDAY, November 5th from 8:30AM-2:30PM. The Mollen Clinic will be offering H1N1 influenza shots to our students. The Mollen clinic is operating at Boulder Creek, but this is not a service of Paradise Valley Unified School District. Please see the Mollen Clinic website for permission slips and more information concerning the H1N1 vaccine at http://www.flushotsusa.com/
Cash, Check or Credit Cards will be accepted. Please note, no child will be turned away for lack of insurance. Any questions should be directed to the Mollen Clinic - phone 480-214-2000 or you can check the website at www.FluShots.USA.com.

Medication reminder

I need your permission to dispense medication to your child. Please complete medication forms when dropping off any medication, forms are available in my office or the front office. Thank you in advance for understanding that, while I enjoy a hand written note from you, it is not in accordance with district policy.
*All over the counter medication must be dispensed through my office, yes, this includes cough drops! All medications need to be in the original container, to ensure that medication dosage is correct and is not expired.
*Doctor Prescribed Medication must be in original container with the prescription label intact with patientÃs name.
I am so looking forward to this school year with your families.
Please remember it is district policy that your child be fever (no fever without the aid of Tylenol or Ibuprophen ). and vomit free for 24hours prior to returning to school. This district policy protects all students from illness. Thank you for your help.
